Western Psychiatric Institute is hiring a Full-Time Sr. Service Coordinator to help support the day to day operations of the WPR - Intensive Case Manage Department for its Champion Commons location. This position will work Daylight Monday thru Friday with on-call on the weekends as needed.
Provides mental health service coordination services, according to the level of need, to a population of serious and persistent mental health patients as described by the most recent Pennsylvania Bulletins regarding Blended Service Coordination, and in accordance with the Allegheny! County MH/MR Plan.
The service coordinator helps pat! ients to gain access to, monitors and coordinates needed mental health and related services and will assist patients to acquire and maintain basic living skills and needs such as housing, food, medical care, recreation, education, and employment, so that the individuals can achieve stability in the community in the least restrictive environment possible. The elements of the practice must A) reflect cultural awareness and B) be based on the principles of Recover, the Community Support Program (CSP) and Child Adolescent Service System Program (CASSP) and C) demonstrate knowledge of service coordination skills, community and service access and an understanding of the clinical implications of the consumers mental illness.
Responsibilities
- Provides service coordination services to a caseload of seriously and persistently mentally ill consumer. Caseload size will be determined by allowable provisions specified in the OMHSAS Blended Service Coordination ! regulations and the Pennsylvania Bulletin.
- Works within the context of the interdisciplinary treatment team and/or as a member of an identified program/module and as such is expected to attend staffing, consultations and case conferences on assigned consumers.
- In some cases the treatment team may consist of the service coordinator and the physician, meaning the service coordinator is required to be able to work autonomously, but in a manner consistent with the consumer's treatment plan.
- Monitors all service received by individuals who are served by the Service Coordination program.
- Reviews cases, meets with individuals' Families, agency Directors, Advocates, Attorneys, school Personnel and attends staffing in or out of the office.
- Assesses and monitors individuals to insure that they are receiving the appropriate level of service coordination services.
- Documents and completes assessments, service plans, recovery plans, p! rogress reports, case correspondence, service logs and other required m! onitoring and reporting documentation as appropriate and on a timely basis.
- Assists consumers to develop plans and goals needed to facilitate independent functioning and to decrease dependency.
- Able to comprehensively assess the holistic needs of the consumer in formulating these plans.
- Ensures individuals being served needs are met through the utilization of natural supports (family, friends), community and generic services and specialized services (MH/MR, OVR, D&A).
- Assists individual and family to identify, link, access and coordinate such resources.
- Plans and goals are to indicate consumer self-determination and to be based on the consumer's strengths, needs, interests and cultural preferences.
- Assists consumers to access and to coordinate appropriate mental health and related services.
- Assists consumers to obtain and maintain culturally appropriate basic living needs and skills such as: housing, food, medic! al care, recreation, education and employment, through referral and assertive follow-up, advocacy and problem resolution.
- Through regular contact with consumers and providers, monitors consumer's participation and satisfaction in all services used by consumers on their caseload, and determines if the individual is progressing and if specific services continue to be needed and are appropriate.
- Convenes, facilitates and participates in interdisciplinary team meetings to ensure appropriateness and responsiveness of services in relation to individual and/or family needs.
- Maintains face to face contact at the minimum intervals as required by the Pennsylvania Bulletin.
- Contact will be more frequent if necessary based upon the needs and/or wishes of the consumer and the treatment team.
- With the permission of the adult consumer, contacts may also be provided to family members, collateral and support persons.
- Face to face contact! s are to take place outside of the traditional office setting; in the c! onsumer's home or in the community.
- Will maintain regular contact with consumers during inpatient hospitalizations.
- Will offer ongoing support to the consumer and consultation to the treatment team. Will seek to be involved in discharge planning within parameters described in the Pennsylvania Bulletin.
- Provides advocacy for consumers, including participation in 302 and other involuntary commitment proceedings, guardianship and custody proceedings.
- Able to identify basic medications and side effects and to supervise the consumer in correctly filling his/her pill box and performing pill counts.
- Also is able to identify common adverse reactions to medication and report to the physician.
- Able to function autonomously and analyze and problem solve moderately complicated problems.
- Serves as a resource and consultant to peers, Institute colleagues and other community providers, as needed.
- Able to work with sel! ect consumers as a physician/service coordinator dyad/team. 70% of the service coordinator's time will be engaged in billable, consumer oriented activity.
- May facilitate or co-lead small groups of consumers/or significant others to increase understanding of the consumers issues or to improve coping skills and performance of activities of daily living.
- Attends agency, state and county training as appropriate or as required.
- Participates in an on call system as required by regulations
- Assist the Supervisor in running team meetings, training new hires and working for the development of the individual team and the department.
- Be the "go-to" resource for the person served and their family.
- Assure that there are effective "safety net" resources for the persons served
- Clearly communicate to the person what they can expect fro! m the system and what the system will expect to them.
- Assure t! here is periodic assessment & cross system planning to meet the needs while utilizing their strengths.
- Prepare for, convene/facilitate service planning meetings and provide follow-up after meetings.
- Assure there is cross system coordination of services and that services are being provided
- Develop relationships that endure with persistent outreach even when there is reluctance to receive services.
- Assist the person served in developing and using natural supports.
- Be a persistent advocate for those served and give feedback on systemic problems.
- Provide a consistent positive outlook which encourages recovery and full inclusion in the community.
- Bachelor's degree in Behavioral Health (Social Work/Psychology), Human Services or related field required.
- Three years of human service experience and working knowledge of human services, paperwork, and PA state regulations f! or targeted case management required.
- Prior experience working in community settings required.
- Safe driving record required.
- May in the course of their duties be in rooms where medications are stored or may transport medications when they transport patients. Transportation of medication is limited from one department to another.
