Position Title: Licensed Practical Nurse - PC
Departmental Title: Nursing
Supervisorâs Title: Nurse Manager
POSITION TITLE DESCRIPTION - SUPERVISOR
The Personal Care LPN is responsible for providing work schedules and assignments to a group of non-exempt employees or a small group of professionals. Employees supervised w! ill have similar occupational skills as the supervisor. May co! nduct performance reviews and process payroll. Recommends, and may conduct disciplinary action and is involved in resolution.
POSITION OBJECTIVE
Also functions as charge nurse in the Star Points Personal Care building. Coordinates all nursing activities on assigned unit during tour of duty.
KNOWLEDGE / EXPERIENCE REQUIREMENTS
1. Graduate of an accredited LPN program.
2. Currently licensed in the Commonwealth of Pennsylvania.
3. One year recent long-term care experience or geriatric care preferred.
4. Demonstrates superior nursing skills.
5. Must have current CPR certification.
6. Must meet physical examination requirements.
7. Demonstrates good team building skills.
8. Basic computer skills preferred.
ESSENTIAL FUNCTIONS
1. Directs the nursing staff on assigned unit utilizing the team approach to resident focused care and supervises the CNA staff and assists ! in the ongoing evaluation of nursing personnel.
2. Performs all specialized LPN services on unit, such as medication pass, Foley insertion, would care, other specialized treatments and physician telephone orders.
3. Performs assessments on all new admissions, readmissions, and as changes in resident conditions dictate. Develops resident support plans per the finding of the residentâs assessment. Provides complete and accurate documentation in each medical record. Reports pertinent data to the Nurse Manager, the Administrator, and next of kin/Power of Attorney.
4. Maintains an accurate and sate system to control all medications including controlled substances and ensures that all medications are available and all drug records are accurate.
5. Reviews resident orders and accurately transcribes physicianâs orders. Prepares paperwork necessary for transfers, M.D. visits and discharges.
6. Makes daily rounds to all residents, moni! tors resident care and the nursing environment, and provides direct car! e as necessary.
7. Evaluates resident needs and effectiveness of nursing care, reviews care and recommends changes in care as needed.
8. Monitors supplies and oversees use of nursing equipment and informs Nurse Manager of supply/equipment needs.
9. Maintains good rapport and communications with co-workers, residents, family, physicians, other facility departments and the community at large. Receives and gives appropriate, accurate and concise reports at change of shift.
10. Participates in in-service and education programs, care evaluation and nursing staff meetings.
