The purpose of such meetings is to assist participants in meeting their recovery needs.

RESPONSIBILITIES:

  • Case Managers will serve as liaisons between the treatment facility and legal team (including Treatment Court Judge, Assistant District Attorney and Assistant Public Defender). As liaisons, Case Managers will inform all parties of their participants’ treatment progress. This includes arrests or any other problematic behavior that might jeopardize their status with treatment court.
  • Case Managers will prepare summaries about specific participants’ progress on selected Wednesdays and present summaries at the bar of the court on those selected Wednesdays.
  • Case Managers will ensure the delivery of comprehensive services to assigned participants in the areas of housing, education, vocation, HIV and recovery oriented community servi! ces to 40 to 50 individuals.
  • The written documentatio! n of services maintained in each participant’s record/chart, in accordance with agency standards, managed care insurance providers and licensing requirements. The utilized format will consist of Data: Assessment: and Plan. Interactions are also to be documented in the Treatment Management Information Systems Database (TMIS). Relevant documentation can include telephone calls, phase progressions, and updates from status listings.
  • Case Managers will prepare graduation packets for participants assigned to their caseloads, completing Phase IV, the Thursday before the graduation ceremony.
  • Case Managers will provide monthly caseload statistics on the Friday following each monthly graduation ceremony, as well as submissions for potential graduates for the following month.
  • Case Managers will assist participants with enrolling in the Department of Public Assistance services.
  • Case Managers will play a key role in developing intra and inter organi! zational linkages, fostering collaborative relationships with various programmatic entities.
  • Case Managers will conduct urine drug screens for Treatment Court participants.
  • Case Managers will regularly meet with participants in treatment facilities, recovery houses and follow up on court ordered sanctions or requests and phase calculations.
  • The Case Management unit will be available to participants, families, treatment facilities, judge and legal team Monday through Friday, 8:30 A.M. to 5:00 P.M.
  • Case Managers are expected to check their email and telephone voicemail throughout their workday.
  • Case Managers must consistently assess each assigned participant’s progress, during individual sessions using the PCPC (Pennsylvania Client Placement Criteria) standards, to ensure their progress is accurately reflected at the PCPC criteria’s level of care.
  • Case Managers will perform all other duties as assigned.

  • Working knowledge of DSM-IV and co-occurring issues
  • Working knowledge of participants with forensic affiliation
  • Bi-lingual fluency a plus
  • Pluses for this opportunity include: case management services with participants with forensic backgrounds who have abused illicit psychoactive substances; and experience in courtroom presentations.

EXPERIENCE:
  • Experience in behavioral health services
  • Must have experience with substance abuse treatment, case management.
  • Minimum of 2-3 years working within the criminal justice system
  • Experience in providing case management services with the chemically dependent criminal justice population a plus.
EDUCATION REQUIREMENT:
Bachelor’s degree reguired!
